How do book and appointment?

Getting started is easy. You can reach us by phone at 905-682-6500 or email info@modernhealthchiropractic.ca to schedule.
If you prefer, you can use our online booking system here.
To reserve your appointment time you will be required to provide a credit card, but this will only be charged in the case of late cancellations or no shows. Once your appointment is booked, you will receive a confirmation email asking you to complete your New Patient Intake & Consent Form. To get the most out of your appointment, please complete this form and submit it online at least 24 hours prior to your visit
Do you offer direct billing?

We are happy to offer direct billing to most insurance companies, however individual plans vary and may not cover the full amount of each service. Please contact your insurance company for specific details regarding your coverage.
If your plan cannot be billed to directly, payment can be made by debit, VISA or Mastercard and you will be provided with an itemized receipt which you can use to submit for reimbursement.
What is your Cancellation Policy?

Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole in the practitioner's day that could have been filled by another patient.
As such, we require 24 hours notice for any cancellations or changes to your appointment. Patients who provide less than 24 hours notice, or miss their appointment, will be charged a cancellation fee of half of the cost of their service.
